agree not to hold Key of Hope, its officers, employees, or other agents liable for any injury, loss, damage, or accident that I might encounter while on one of their mission trips. I realize and acknowledge that my participation on a mission trip to a foreign country includes many risks and possible dangers. I am well aware that my travel to such a foreign country exposes me to such risks as illness, crime, accidents, disease, war, political unrest, injury from construction projects, and other calamities.
I hereby assume any such risks that might result from my travel to a foreign country, and I
unconditionally agree to hold Key of Hope, its officers, employees, or other agents blameless for
any liability concerning my personal health and well being, or any liability for my personal property
that might be lost, damaged, or stolen while on a mission trip.
I give permission to the designated trip leader to
authorize any emergency medical treatment by certified medical personnel that may be needed due to accident, illness, or injury in the case that I am not physically capable of authorizing such treatment. I understand that reasonable attempts will be made to notify my family prior to any medical treatment being administered.
In the event of my death outside the United States, a family member, or a representative of the U.S. State Department or U.S. Embassy is to be instructed by the following (please check one):
